Details & Overview
To take advantage of this limited time rebate offer, order qualifying Bay Area
Internet Solutions, Covad SDSL services within the specified
order dates. After your service is installed, go to http://www.netopia.com/equipment/promo/rebate/covad_b/
and select your order timeframe to complete, print, and submit the
appropriate rebate form by the applicable postmark date. Please review
the rebate forms on the Covad Rebate Redemption site for information and
rules regarding individual services and rebates.
One rebate per Customer per Service ordered. Customer must pay the first
invoice to qualify for a rebate. If Customer disconnects during the
first year of service, Bay Area Internet Solutions may bill Customer for the rebate
amount. Covad reserves the right to reject any rebate requests that (a)
are incomplete, (b) are duplicative or (c) Covad deems fraudulent.
Rebate checks not cashed with ninety (90) days will be cancelled. Please
see specific rebate offer for additional terms.

Am I eligible for any rebates or promotions that are available?
To take advantage of any promotions, simply review the details, terms
and conditions and follow the submission instructions. Qualification
for rebates will be detailed in the terms and conditions.
How do I find my Circuit ID number?
Your Circuit ID number will be sent via email notification from Bay Area
Internet and Covad once the order has been submitted. You may also
contact technical support at 800-495-0092 for your Circuit ID.
How do I find my Covad Order Number?
Your Covad Order Number is sent via email notification from Bay Area
Internet and Covad when your order has been submitted. If you do
not have your order number please contact technical support at
800-495-0092 for your Covad Order Number.
How do I find my router name (CPE) and model number?
The router name and model number can be found on the bottom of the
router itself. For example: "Netopia 4562T".
Do I have to use the installation street address?
Yes, checks will only be issued to the customer and company named on the
order and mailed to the address where the Covad service was installed.

How do I check the status of my rebate online?
Log on to the Netopia rebate web-site at http://www.rebateshq.com - you can enter your
customer submission number (provided when you pre-qualify for rebate
online) and track the real time status of your rebate.
Is this offer good only in the U.S.?
Offer is valid for U.S. residents only. Additionally, the offer good
only in those states in the U.S.A. where it is not prohibited, taxed or
restricted by law.
